• Pupils and students from 15 till 26 years of age:
  A corresponding photograph of the applicant size 35 x 45 mm, a student form confirmed by school (free at the sales points or on http://www.dpmo.cz/forms/2008-studentska-zadanka.pdf) and the identification card
• The full disability or old-age pension users:
  A corresponding photograph of the applicant size 35 x 45 mm, announcement about the increase of the pension or acknowledgement of the old-age pension or the state authority´s confirmation of the pension dated not longer than 2 months ago. The identification card.
• Citizens over 70 years of age:

• Workers of the organization Charity:
